To be included in MIS, those who have allowance card should contact the concerned City Social Service / Upazila Social Service / District Social Service Office / Councilor / Union Parishad office along with the disability ID card. –

    "Rights and Protection of Persons with Disabilities Act 2013" has been enacted to improve the quality of life and overall welfare of the disabled population. Section 31(1) of the Act provides for the registration and issuing of identity cards to persons with disabilities. To that end, regular survey activities are being conducted starting from 2013 to identify the type of disability, determine the extent and determine the correct statistics of the disabled population. Disability Information System (www.dis.gov.bd) database with web based application has been prepared to collect information of persons with disabilities which is being updated regularly.
    Following instructions are to be followed for online application for obtaining Disability Identity Card (ID Card):
     
    1. Applicant must be a citizen of Bangladesh.
    2. Only persons with disabilities, their guardians or any organization on behalf of persons with disabilities shall use this site.
    3. Applicant must have birth certificate or national identity card.
    4. In the application form, the signature of the applicant should be scanned and placed in place of the signature.
    5. Must have valid mobile number and e-mail address.
    6. The online application form will be submitted to the concerned upazila/city social service office at the current address of the applicant.
    7. Must appear for the medical examination as per the time given by the concerned upazila/city social service office.
    8. The printed copy obtained after submitting the online application form should be brought along while appearing for the medical examination.
    9. After receiving the SMS from the concerned upazila/city social services office, the identity card (ID card) should be collected.
